The Ottawa Valley Family Health Team Commitment
We are committed to ensuring that our patients have access to timely, high-quality primary care medical services while emphasizing health promotion, disease prevention, and improved management of a chronic disease. A group of physicians from the Almonte Family Health Network applied to the Ontario Ministry of Health and Long-Term Care in the summer of 2009 to become the Ottawa Valley Family Health Team. The Ministry approved the application in December 2009.
Our Services
In addition to family physicians, the Ottawa Valley FHT includes nurse practitioners, registered nurses, registered practical nurses, pharmacists, mental health/psychotherapists, and dietitians.
Primary Services
Primary care generally refers to a patient’s first contact with the health care system. It is the point where people receive care for most of their everyday health needs. The Ottawa Valley FHT has 29 primary care physicians and 3 primary care nurse practitioners.
Uninsured Services
Not all of the services that your primary care physician provides you are insured through the Ontario Health Insurance Plan (OHIP). Your physician can request a payment from you for those services. A listing of the most common uninsured medical services, including the associated costs, which are not covered by OHIP is available at the clinic.
Mental Health Supports
At the Ottawa Valley Family Health Team, we understand the importance of mental health and provide comprehensive support to our community. Our team of experienced physicians and interdisciplinary health providers are dedicated to serving the needs of Mississippi Mills, Carleton Place, Beckwith, and the surrounding areas. We offer a range of mental health services, including counseling, therapy, and support groups. Whether you’re dealing with stress, anxiety, depression, or any other mental health concern, we are here to help. Chronic Disease Management and Prevention and Health Promotion
As part of the comprehensive care we provide to patients and their families, we also offer chronic disease management and prevention and health promotion programs.
Chronic disease management focuses on preventing diseases from progressing following diagnosis. Health promotion focuses on maintaining health and preventing diseases from occurring.
The Ottawa Valley FHT offers chronic disease management and health promotion programs and services in the following areas:
- Diabetes
- Geriatrics
- Respiratory Diseases/Smoking Cessation
- Mental Health
- Women’s Health/Maternal and Postpartum Care
Patients who are eligible to participate in these programs are referred by their family physician.

Diabetes
Ottawa Valley Family Heath Team’s Nurses and Dietitians meet with you and help you understand how your diabetes can best be managed. This is the time to have all of your questions answered.
Circumcision Information for Parents
